The attached model template filing document is for every witness to follow the filing methods shown in the PDF. They have to check with the LEAP APP Dashboard and also verify their manual count of parents and teachers they found, and fill in the Final measurement column. Parents + Guardians count can be written in the parent total number, and the attendance of the teachers and counted to determine the total number of teachers. Also, make sure to write in the prior experience that you have been briefed on your duties for the GWR witnessing.

The second PDF is the form where you will be doing the actual submission. This form you will be downloading from the LEAP App and filling in manually, signing, and submitting to the Head Master to upload the digital copy and then forward the print paper to the MEGA PTM Control Room.
